FGDC Keyword Textbox Not Visible

This Error Reference applies to: 2.0.2

The metadata entry form does not alow addition of keywords. The Add Element (+) opens a new item but there is still no text box.

I've noticed similar problems with FGDC data for

Online Linkage for the citation as well as for the metadata

Has anyone else noticed these issues or found a work around?
